what to expect spinal
As a prospective spinal patient the first appointment is to determine what is wrong and if you are a chiropractic patient. Intake paperwork can be sent online or can be downloaded to the right. After we go over the paperwork, I develop a time-line of your health and complete a physical exam based on it. If xrays are determined to be necessary they will be taken at this time, and then 11 points of anatomy are evaluated to determine your posture. This completes your first appointment with some homework. The cost for the first appointment is $60-$200.
The second appointment is where you find out what is wrong, and what we are going to do to fix it. If you have been determined to be a chiropractic patient and agree to the phases to fix your problem I will administer the first alignment of your care plan. The cost for the second appointment is $40 for the Report of Findings plus $50 for the alignment.
what to expect nutritional
As a prospective nutritional patient the first appointment is to determine what is wrong and if you are a nutritional patient. Intake paperwork can be sent online or can be downloaded to the right. After we go over the paperwork, your health data will be collected . a time-line of your health will be developed and a physical evaluation will occur. This completes your first appointment with some homework. The cost of the first appointment is $135.
The second appointment is where you find out what is wrong, and what we are going to do to fix it. If you have been selected to be a nutritional patient and agree to the phases to fix your problem I will determine your supplement dosing and you will purchase the supplements needed to improve your health. The cost for the second appointment is $50 plus supplement cost.

Aligned Care Chiropractic is a fee for service company. However, if you receive out of network benefits, documentation of services can be provided to submit to your insurance carrier.
Cash, check, credit/debit cards, and health and wellness account cards are accepted as payment at the end of each visit.

Forms typically are sent via email after scheduling. If you don't have email please print and complete the required forms for your first appointment. If you don't have email and are unable to print the forms please show up 20 minutes prior to your appointment to complete the forms.
